Money is a vital component for a sustainable and harmony life. Knowing the main function of money in marriage is important for a successful marriage. According to (Loh,2012) a research by Smart Money magazine, cited by shows that 70% of spouses surveyed discuss about money no less than one weekly while 36% of men and 40% of women confess that they lied to their couple about their spending. Many couples do not aware about the importance of money. The ability to pay household and utility bills, the tendency to ensure the children get their rights as well as the chance to have a comfortable and sustainable living are therefore, the importance of money in marriage. One of the reasons why money is important in marriage is to pay household and utility bills. Just like how organizations have their organization’s financial details each year to trace how they spend their money to pay for things married couples also need to record their expenditure and examined them monthly. (Eugene, 2009). A couples need to distribute their money to pay the energy, electrical and telecommunication bills. Electricity is necessary to run all home utilities such as refrigerator, television, radio, including hot water heaters and water pump that allow toilet to work. Besides electrical bills a telecommunication bills also need to be managed well so that we can ensure smooth communication between a family members. The example of communication utilities is such as encompass telephone, satellite television service and Internet service. For instance communication utilities such as internet can be used by the family members especially children who are still study to find information, communicate with their friend, and sharing their opinion. A married couple also need to allocate their money for the household food budget. The monthly budget for the food and groceries of the house should be managed accurately. â€Å"It is significant to give them with a healthy-balanced diet and encourage them to work out frequently† (Vijaindren, 2012). Parents should aware that your child needs a variety of food to fuel his growth, strengthen his immune system and support his brain and nervous system. So it can be conclude that money is important in marriage to have a healthy lifestyle because an apple a day can keep a doctor away. Another importance of money in marriage is to ensure children get their rights. A beneficial education is a necessary for children (Rosmah, 2010). Every child should have an adequate education to secure their future. Money can be used to pay necessary fees regarding the education or study. For example, money can be used to pay an education fees for children who studying at primary, secondary or even children at the college. As an example, money is useful for buying a basic need for children education such as school uniform, stationeries, and of course a books. Every child deserved an additional education such as home tutor or tuition. With money, tuition fees and additional books can be paid consistently. According to Rosmah (2010, as cited in Lim (2010) a marriage that has consistency in money should give their children a well-nourished life. Every parent can make their children more valuable and comfortable with money. For example provide a comfortable shelter such as personal room for their uses. Furthermore with money parents will have the tendency to ensure that children get their rights. As an example, children nowadays will request for an up-to-date gadgets, such as smart phone, tablet, or even an android phone. This can be fulfilled with money. Thus, money is important not only for an adult, but can be very useful for a sustainable in children’s life. One of the other reasons why money is important in marriage is to have comfortable and sustainable life. For instance one of the family members having any major health problem such as cancer or heart problem that need to spend a lot of money to heal at least this scheme can support it. Another importance of money is to keep away the future cohort to cope with difficulty and struggle to have a sustainable and comfortable life. Life insurance gives a person chance to propose a consistent financial for his or her family in the event of disaster or poor health problem happen to the family members (Morris, 2010). With the power of money a couple can buy a life insurance for their house, vehicle and family member. For example when a giant Tsunami hits Japan on 2012 many facilities and infrastructure has been demolished, at the same time many victim had lost their family members. So the reason why you need to take out an insurance policy is to look after who’s left behind you especially the ones that you loved. In a nutshell, the importance of money in marriage include chance to have a comfortable and sustainable life, the tendency to ensure the children get their rights as well as the ability to pay household and utility bills. William Wordsworth's poem Upon Westminster Bridge is a sonnet, it creates a pleasurable passage that is easily read and understood while still accessing a great deal of emotion and image form. It gives different readers, many different interpretations of what the poem is about, the images and emotions felt, yet still maintaining the secret of what Wordsworth himself would have had in mind about the meaning of the poem. Upon Westminster Bridge creates for the reader that sense of awe that was felt by William Wordsworth whilst gazing upon the view of London and this awe can also be felt by the readers themselves. This sense of awe can be seen from the very beginning of the passage, â€Å"Earth has not anything to show more fair:† just the language used is like a spell cast upon the reader giving off a sense of calmness and tranquillity. This feeling in the language is reflected in the atmosphere of the time in which the poem is set, early morning, just as the sun is starting to show above the horizon. Where the citizens of London are still in bed, the busy city still asleep and motionless. The impression of religious symbology can also be felt in this poem. â€Å"Dull would he be the soul who could pass by†¦Ã¢â‚¬  the word â€Å"soul† in this line gives the reader the impression of a reverential tone. Soul is a word that is used mostly in a religious context and having it in the poem lets off the feeling of the peace, and heavenliness. â€Å"Dear God! the very houses seem asleep†¦Ã¢â‚¬  this can be taken as an emphasis on the reverential tone of the poem. â€Å"Dear God!† this reference to God fits in and confirms the â€Å"soul† and heightens the religious atmosphere. The first stanza in this poem is like an opening to the rest, an appetiser. â€Å"A sight so touching in its majesty:† The use of sight, so, its, majesty, is to put emphases on the soft sound of the â€Å"s†. This softness is linked to â€Å"touching†. This magnificent view is only softly touching him, the poet, with all this to take in he hasn't yet absorbed the full beauty. This can be seen as one of the most important and meaningful lines in this passage, it uses stillness and serenity, creating the mood, and linking the setting to the poet's feelings at the time. William Wordsworth uses similes in this poem to connect a lifeless thing like the city of London, to humanity, and the natural world to create a unity of all three. â€Å"This City now doth like a garment wear The beauty of the morning†, humans wear clothing, gowns to make us look more beautiful. What this line is saying is that the city wears the morning like a beautiful gown, and the morning is making the city look more splendid, giving it the connection to humans and making it come to life. The use of words with short syllables can make the reader feel informed. It helps capture a huge amount of detail in very little words. â€Å"†¦Ships, towers, domes, theatres and temples lie open unto the fields, and to the sky†¦Ã¢â‚¬  looking at this the reader would start slow and read faster and faster as the list progresses, this is the influence of short syllable words. This paints a mental image in the reader's head as if watching a movie, an elaborate scene with beautiful scenery, and the camera pans across slowly at a constant rate capturing every flower, every tree, hill and a small bee flying across the screen. â€Å"†¦Open unto the fields, and to the sky.† â€Å"In his first splendour valley, rock or hill† and amongst all this artificial beauty of towers and ship the beauty of nature still manages to show through and enhance the whole image. The last stanza raps up the whole of the poem; it recreates the mood of awe and also puts in a little shock. â€Å"Dear God! the very houses seem asleep†; this time the â€Å"Dear God!† is used in a different way. It serves the purpose of heightening the religious feeling, but also heightens the awe and pushes it into shock. It is used almost in a blasphemous way; using God's name in vain. William Wordsworth's appreciation of beauty is revealed not only in the images and similes he chose to use, but also in the gracefully modulated sentences. The rhyming of the last word in the first and last stanza reinforces the reverence Wordsworth felt all his life to the God he understood to be in all nature. Wordsworth's personality and poetry were deeply influenced by his love of nature, romanticising what he saw in the natural world.

As stated before, Miller thought that the common man was the most eligible person with which he could create a tragic hero. With this in mind he set out to make his characters tragic heroes, giving them what he called â€Å"a tragic flaw†, a failing that is not peculiar to grand or elevated characters, nor is it necessarily a weakness. In Miller’s plays, these flaws are not major cracks in their characters, they are on the contrary are something as simple as having too much faith in a system that lets them down when they both need it. This philosophy of Miller, gives us a lot of insight into the characters, and also their actions. Joe Keller, the protagonist, is a man who loves his family above all else, and has sacrificed everything, including his honour, in his struggle to make the family prosperous. His fundamental flaw however, is that he is not a man who sees the bigger picture of life, that the whole of mankind is a †family† that deserves just as much consideration, if not more, than his own family. He also feels as though the †system† will protect him, seeing as he was able to trick it. It is this, that leads to the death of the 21 pilots, that were flying faulty planes made by his factory. The American way of life is clearly seen as his ultimate goal, as he is only looking out for himself and his family. His wife, Kate, is a woman that has a tremendous capacity for love, which I seen no only in her love for her lost son, but also in that it extends to her neighbors, notably to George. It is easy to assume that Kate’s persistence in declaring her son still living, is because she could not could with the idea of her son being dead. This is not so, it is in fact, that her sons death would be a punishment for Joes crime, which would be intolerable for her, so she continues to support Joe’s deceit. Kate seems to the reader to be a very unbalanced and somewhat stupid woman, however she, like Joe, is simply trying to hold the family together, even if it is somewhat desperate. Second Section: Death of a Salesman Death of a Salesman is widely regarded as the ultimate tragedy, and was extremely well received by American critics. It is a complete and utter criticism of the American dream, and the American people of the forties. More pertaining to the concept of tragic heroes however, the play deals with painful conflicts within the family, and how a man can fall, ungracefully from a position of respect within the family. The main character of the play, is the father of the Loman family, Willy. He works as a door-to-door salesman, and travels a lot for his job. His wife Linda, seems to be the strongest person in the family, and supports Willy, who is fed up of working, and is slightly disillusioned about life in general. Biff, the oldest of their two sons, is visiting the family, having been off ‘finding himself’, working on and off, but still not settled down to a decent job. His brother, Happy, is young, and as his name suggests, is the most cheerful person in the family. It is extremely hard to outline the plot of the play for two reasons: the first reason is that not a lot actually happens in the play, it is more about the relationship between characters, and with themselves. The second reason is that it is divided in two ‘sections’ that run parallel to each other in Willies mind: the present, and the past (Willy’s memories). Willy has numerous flashbacks during the play, mainly where he describes an event which had a strong impact on his life. One example of such a flashback would be when Linda is trying to comfort him, and suddenly he is a hotel room with his mistress. The play is mainly a record of the problems that a man suffers in a capitalistic and pushy society. It is set in a pleasant suburban area, in the late forties. The reader mainly follows Willy around, through past and present and by this method of narration, we learn a lot about Willy, through his flashbacks, such as his affair with ‘The Woman’, the fact that he was fired from his job. Whole Exome Sequencing in Inherited Endocrine Disorders Background Molecular diagnosis is important in the management of various paediatric endocrine disorders including disorders of growth, metabolism, bone, hypoglycaemia and sexual development. Traditional PCR-based Sanger sequencing is the mainstay format for molecular testing in paediatric cases. However, the large number of gene defects associated with the various endocrine disorders renders gene-by-gene testing increasingly expensive and unattractive. The large number of potentially relevant genes makes it challenging for hospital molecular diagnostic laboratories to offer gene-based testing of all candidates. Given the high costs associated with single-gene tests, the selection of candidates for single-gene sequencing tends to be sequential rather than inclusive and parallel. In practice, different genes may be outsourced to different clinical or in some cases academic research laboratories which adds to the complexity. Using new high-throughput sequencing technologies, whole genomes, whole e xomes or candidate-gene panels (targeted gene sequencing) can now be cost-effectively sequenced for endocrine patients. In the near future, protocols involving next-generation sequencing would probably be considered as an appropriate component of routine clinical diagnosis for relevant patients. Defects of pituitary hormones lead to abnormalities in growth (e.g., short stature), sexual development, fertility, stress response and other metabolic processes. A number of genes coding for transcription factors have been identified, mutations in which cause medical disorders in humans associated with pituitary deficiencies [1-2]. Some of these factors, such as PROP1, TPIT, POU1F1, LHX3 and LHX4, play roles in the normal embryological development of the anterior pituitary. Mutations in these genes can lead to multiple pituitary hormone deficiencies and/or syndromic hypopituitarism [3]. The transcription factors such as HESX1, OTX2, SHH, SOX2 and SOX3 are involved in midline development. Mutations in these can cause septo-optic dysplasia or holoprosencephaly, both of which may include pituitary hormone deficiencies [4]. Other genes encode the precursors to pituitary hormones (growth hormone, ACTH [through processing of POMC], gonadotropic-luteinizing hormone and follicle-stimulating hormone, and thyroid-stimulating hormone). Mutations in these genes lead to phenotypes characteristic of individual hormone deficiency. The pituitary secretory cells themselves respond to signals originating in the hypothalamus, some of which are also peptide hormones with specific receptors expressed on the responding cells; mutations in these genes or their cognate receptors can also cause combined or specific pituitary deficiencies [1]. However, many cases of congenital hypopituitarism still remain unexplained and most are presumably due to other causes, either mutations in other deve lopmental genes or epigenetic influences during embryogenesis. Short stature is a common presentation to the paediatric endocrinology clinics. However, no cause is identified in a large proportion of patients who are classified as having idiopathic short stature [5, 6, 7]. It is estimated that the underlying cause for short stature remains unknown in approximately 80% of patients [8]. In a large-scale pooled Next-Generation Sequencing study to identify genetic causes of short stature, 4928 genetic variants in 1077 genes were present in patients but not in control subjects [9]. Large-scale sequencing efforts have the potential to rapidly identify genetic aetiologies of short stature. In another study, seeking to identify known and genetic causes of short stature by conducting whole exome sequencing of the patients with severe short stature and their family members, genetic cause of short stature was found in 5 out of the 14 recruited patients [10]. Rare genetic defects in the GH/IGF-1 axis have been found to cause short stature. A higher frequenc y of rare CNVs (common number variants) has been reported in patients with short stature [8, 11]. A recent study to define genetic characterisation of a cohort of children clinically labelled as Growth Hormone or IGF1 insensitive found that whole exome sequencing contributed to the diagnosis of children with suspected growth hormone and IGF1 insensitivity, particularly in the Growth hormone insensitive subjects with low serum IGF1 SDS and height SDS [12]. It may be now possible to identify likely genetic causes of short stature by implementing genomic investigative techniques like whole exome sequencing in many of these children who have unknown reasons for their poor linear growth. Congenital Hyperinsulinism (CHI) is the most common cause of persistent and recurrent hypoglycaemia in infancy [13]. It is the result of unregulated insulin secretion from the pancreatic ÃŽ ²-cells leading to severe hypoglycaemia [13, 14]. This condition has been reported in nearly all major ethnic groups and affects at least 1/50,000 children of European descent [14]. CHI is caused by genetic defects in key genes regulating insulin secretion. The genetic basis of CHI involves mutations in nine different genes (ABCC8, KCNJ11, GLUD1, GCK, HADH, SLC16A1, HNF4A, HNF1A and UCP2), which regulate insulin secretion from the pancreatic ÃŽ ²-cells [14,15]. The most common molecular cause of CHI is the dysfunction of the pancreatic KATP channel encoded by the sulfonylurea receptor gene (ABCC8) and the inward rectifying potassium channel gene (KCNJ11) [14,15]. CHI can also be secondary to risk factors like birth asphyxia, intra-uterine growth retardation, Rh isoimmunisation and maternal diabetes mellitus or associated with various developmental syndromes [16]. Histologically, CHI can be associated either with diffuse insulin secretion or with focal adenomatous hyperplasia. Positron emission tomography scan using Fluorine-18 L-3, 4-dihydroxyphenylalanine (18-fluoro DOPA-TC-PET-scan) has been used to distinguish focal from diffuse forms. Medical treatments of CHI include diazoxide (KATP channel activator), somatostatin analogue (octreotide) injections, and appropriate diet. The surgical treatment with subtotal pancreatectomy is required in diffuse CHI when medical treatment and dietary therapies are ineffective, whereas focal CHI can be cured with resection of the focal area of adenomatous hyperplasia [14, 15, 16]. Recently, mammalian target of rapamycin (mTOR) inhibitor sirolimus has been used in treatment of persistent severe CHI not amena ble to medical therapies [18]. CHI has been described as an associated finding in various syndromes like Beckwith-Wiedemann, Kabuki, Trisomy 13, Mosaic Turner, Sotos, Usher, Timothy, Costello, Central Hypoventilation syndrome and Leprechaunism (Insulin Resistance Syndrome) [17]. However, in many patients, with clinically defined syndromic features and with hypoglycaemia, no identifiable genetic cause contributing to hyperinsulinism is found. In a large series of 300 patients, genetic diagnosis was made only in 45.3% of the patients and mutations in ABCC8 were the commonest identifiable cause [19]. The vast majority of patients with Diazoxide-responsive CHI (77.6%) had no identifiable mutations, suggesting other genetic mechanisms [19]. Molecular diagnosis can be very important for clinicians to manage the patients more effectively and to counsel parents on the prognosis and disease recurrence. Whole Exome sequencing can be advantageous in these groups of patients to identify the mol ecular defects and to assess the coding variants that may be pathogenic in these patients [20]. Aims To identify novel genetic causes of rare inherited endocrine disorders in children with a focus on congenital hyperinsulinism, short stature of unknown etiology and IGF1 abnormalities by using whole exome sequencing. Experimental Design and Methods Patient Recruitment Patients with a diagnosis of CHI referred to Alder Hey Children’s Hospital, which is a national referral centre for CHI, will be recruited into the study. A written informed parental consent will be obtained. These patients will be biochemically confirmed as CHI using the following criteria: Blood glucose concentration of less than 3.0 mmol/l with detectable insulin and/or C-peptide Glucose requirement > 8mg/kg/min Low levels of ketones and fatty acids during the episode of hypoglycaemia Clinical and biochemical data will be collated from referral letter or by case note review. Patients with a secondary cause of CHI such as perinatal asphyxia, intra-uterine growth restriction, Rhesus isoimmunisation, infants of diabetic mothers and infants with Beckwith Wiedemann syndrome will be excluded from the study. Patients are considered to be unresponsive to medical treatment if recurrent hypoglycaemia episodes ( Patients attending the Paediatric Endocrinology clinic at Alder Hey Children’s Hospital with severe short stature (>3 SDS below mean) for age and sex in whom the standard clinical work up has not revealed a diagnosis for their short stature will be recruited into the study. Patients referred or evaluated for growth hormone insensitivity (growth failure, low serum IGF1 and normal/elevated serum GH) or IGF1 insensitivity (pre- and postnatal growth failure associated with relatively high IGF1 levels) will also be recruited into the study. A written parental informed consent will be obtained prior to the recruitment. Whole Exome Sequencing (WES) WES will be performed at the Centre for Genomic Research (CGR) based at the University of Liverpool. The test will be ordered after explaining the risks and benefits of testing to the patient and obtaining written informed consent. Each patient (and their parents or guardians) will be advised of the potential disclosure of conditions unrelated to the indication for testing that might warrant treatment or additional medical surveillance for the patient and possibly other family members. Peripheral-blood samples will be obtained to isolate DNA from the patient and both parents where possible. High-throughput sequencing will be performed using Illumina HiSeq2500. The genomic DNA samples from probands will be fragmented, ligated to Illumina multiplexing and amplified by means of a polymerase-chain-reaction assay with the use of primers with sequencing barcodes. Variants that were deemed clinically significant will be confirmed by means of Sanger sequencing. Parental samples, if available, will also be analysed by means of Sanger sequencing. Further functional analysis will be undertaken to establish the pathogenicity of the identified variant by utilising the in-house lab facilities at Institute of Child Health. Relevance of the proposed project Despite the advances in understanding the molecular pathogenesis, specific genetic determinants are not known in nearly 50% of patients with CHI and 80% of children with short stature. Whole exome sequencing in this group of patients will help to understand and identify the potential causative mutations in genes implicated in insulin regulation and growth. This will help the clinicians to provide optimal treatment and to counsel patients on disease progression and recurrence risk. Identification of novel genetic aetiology has the potential to identify novel therapeutic strategies for these patients. The applicant will spend time initially at Alder Hey Children’s Hospital to recruit patients and then at Cincinnati Children’s Hospital to learn the techniques of WES, bioinformatics and functional analysis. The applicant will then return to Alder Hey Children’s Hospital Institute of Child Health, University of Liverpool to complete the study and strengthen the local research expertise relevant to next generation sequencing.
